To receive a Vitamin D injection, patients must provide laboratory results showing their Vitamin D level from bloodwork completed within the past three months.

Before your appointment
To ensure a safe and seamless experience, the following must be completed prior to your visit:
• Medical screening form
• Consent form
These forms must be completed at least 24 hours before your appointment. If they are not completed in advance, your visit will need to be rescheduled.

• Vitals are taken if required
